Job Description :
Currently, our rapidly expanding team is in need of Diabetes Educator/Registered Nurse who can work for us on full-time basis. This position is for a 12 month contract working at our Bayview site and within our Clinical Research Unit (CRU).

Responsibilities include but are not limited to:
Instruct patients on diabetes self management skills in both individual and group settings
Obtain relevant health history of patients
Develop a care plan for patients with diabetes(Type 1, 2, GDM, Pre-Diabetes)
Teach administration of insulin effectively, including titration and management as per medical directives
Teach self-monitoring of blood glucose (SMBG) effectively
Assess patient readiness for insulin pump therapy
Manage patients on insulin pump therapy, as per medical directives
Perform patient follow ups in person, by e-mail or by telephone
Contribute to the development of patient handouts, as required
Provides consultation as requested to community groups, partners and team members
Mentor volunteers, as required
Effectively communicate to patients, families, caregivers and/or healthcare professionals
Employ information technology appropriately for patient care (i.e. statistical tracking)
Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment
Facilitates appropriate in-house and external education sessions
Perform clinical skills for the Clinical Research Unit (CRU) i.e. IV pump management, drug dispensing, and patient care

Requirements include:
Bachelor in Nursing
Current registration and in good standing with the College of Nurses of Ontario
Certification as a Diabetes Educator (CDE) or willingness to obtain
Must have at least one year experience at a hospital or medical clinic setting
Must have experience teaching/counseling one-on-one or groups of people
